Effects of Cocaine Mixed with Fentanyl in the Philadelphia & Wilmington Area

The drug supply in the Philadelphia and Wilmington (Delaware Valley) region is uniquely dangerous due to the widespread adulteration of cocaine and fentanyl with veterinary sedatives. This mixture, often consumed unknowingly or intentionally as a “speedball,” has created a specific public health crisis characterized by two main phases of adulterants: Xylazine (“Tranq”) and, emerging in 2024, Medetomidine (“Rhino Tranq”).   •   “Flesh-Rotting” Wounds (Xylazine): A defining and horrific effect in this region is the prevalence of necrotic skin ulcers caused by xylazine. Unlike typical injection marks, these wounds cause rapid tissue death (necrosis) that can expose bone and tendon. They can appear anywhere on the body—not just at injection sites—and have led to a significant rise in limb amputations.(2) (3) (4) (5)
  •   The “Tranq Nod” & Vulnerability: The combination of fentanyl (an opioid) with these sedatives creates a profound, blackout state of unconsciousness. Users are often seen standing or sitting in a hunched, trance-like stupor for hours. This leaves them highly vulnerable to robbery, assault, and exposure to the elements.
  •   Naloxone Resistance: While Naloxone (Narcan) can reverse the respiratory failure caused by fentanyl, it cannot reverse the sedation from xylazine or medetomidine because they are not opioids. (6) Rescue efforts are complicated; a victim may have their breathing restored by Narcan but remain comatose due to the sedative, requiring hospitalization. (7)
  •   Medetomidine Toxicity (2024–2025): By 2024, the even more potent sedative medetomidine began replacing xylazine.(8) It causes severe bradycardia (dangerously slow heart rate) that can stop the heart even if the person is breathing.(9) It is also associated with a complex withdrawal syndrome involving extreme anxiety, hallucinations, and dangerous spikes in blood pressure.

National Death Toll During the Biden Administration

From January 2021 through January 2025, the total number of drug overdose deaths in the United States was approximately 405,000. The vast majority (over 70%) of these deaths involved illicitly manufactured fentanyl, often mixed with other substances like cocaine, methamphetamine, and counterfeit pills. While the administration saw record-breaking death tolls in its first two years, 2024 marked a historic turning point with the first significant decline in overdose deaths in decades.
